李嗣真（？ - 696年），字承胄，邢州柏人（今河北省隆堯縣）人，一說滑州匡城（今河南省長垣縣西南）人。博學曉音律，兼善陰陽推算之術，唐朝時期大臣、書畫家。李嗣真在弱冠之年，舉明經，補許州司功。唐高宗時，直弘文館，與劉獻臣、徐昭同為弘文館學士中「三少」。出為義烏縣令、始平縣令，入為太常丞。高宗看重他的人才，征拜為司禮丞，掌管五禮儀注，加中散大夫，封常山子。永昌元年（689年），授御史中丞、知大夫事。後被酷吏來俊臣陷害，出為潞州刺史，流放嶺南藤州，很久之後征還，途中在桂陽（治今湖南郴州）暴卒，謚昭。善書畫評論，著有《明堂新禮》、《詩品》、《書後品》，《續畫品錄》。
